OSHA requires that employers notify them when an employee is killed or suffers a work-related hospitalization, amputation, or loss of an eye. Fatalities must be reported in 8 hours and all other accidents resulting in hospitalization within 24 hours.

MOSH (Maryland Occupational Safety and Health Administration) does not allow online reporting.
Virginia Employers
Virginia requires that all fatalities and hospitalizations, amputations or loss of eyes be reported in 8 hours! This is more stringent then the federal requirement. See Virginia rules here.
